Request a prescription from your child’s doctor for a child or adolescent referral, and Primary Care Physician for adult referral. ( See below for more details)
Requesting a Prescription for Speech Therapy

Call your child’s primary care physician and state that you are having concerns and would like a referral for speech language therapy evaluation.

Ask the doctor’s office to fax the prescription to (844-315-9608).

The prescription will need to have an order for a speech language evaluation. Diagnostic Code. Codes typically used for Speech Language Evaluation include:
