No insurance? No problem. Now offering comprehensive speech and language evaluations for private pay clients. A speech therapy evaluation will consist of parent/patient interview and past medical history intake, formal/informal assessments and observation to include standardized testing to determine the presence or absence of a speech, language, feeding, cognitive or swallowing disorder. Cancellation Policy
To ensure we can use our time efficiently, we require the following information prior to evaluation: Step 1: Prescription from your primary care physician for evaluation and treatment AND Most Recent Hearing Test Results or NEWBORN Hearing Test Fax to 844-315-9608. Step 2: Complete & Submit Intake forms - Forms will be sent from Fusion. Step 3: Upload a copy of your Government issued ID & Front of Insurance Card to the intake form. Step 4: Upload previous evaluations (speech-language, audiological, psychological, Most Recent Copy of IEP), if applicable Step 5: Once all these items are received, we will contact you via EMAIL to schedule your initial visit within 48 hours. In order to schedule your initial evaluation, you MUST schedule a booking time at www.SovereignTherapy.com. Click on the BOOKING Tab to Book a new patient evaluation. If you fail to complete your file, in accordance with HIPAA regulations, your information will be deleted from the system after 14 days if your initial visit has not been scheduled.
